Instructions for Adobe Acrobat Reader Users

When completed just click on the print button and sign. Bring in your signed form along with your pet’s medical records prior to or at check-in. You will not be able to electronically save the form to your computer. If you prefer to hand write out the form, simply open the form and click on the print button.

Note to Adobe Acrobat Full Version Users:

You should be able to digitally sign and save your filled-out form to your computer (save as .pdf). After saving the form to your computer, you can email it directly to us at services@pawprintinn.com. Thank you.
